    Voluntary Term Life Insurance

    Need additional coverage for you and your family? Consider Voluntary Term Life Insurance for you, your spouse/domestic partner or children. You pay the full cost of coverage and your premiums are after-tax. This means that you will not be taxed when the benefit is paid to you or your family.

    You can elect the first level of coverage when first eligible without submitting a Statement of Health.
    • You can only increase coverage by one level each year with proof of good health.
    • Your spouse/domestic partner must provide proof of good health to purchase any level of Spouse Life.
    • Coverage requiring a Statement of Health becomes effective only after MetLife's approval and you have paid your first premium.

    Provide extra security for your family in case of an accidental injury or death with Voluntary Personal Accident Insurance (VPAI). You pay the entire cost of coverage and your contributions are after-tax. This means that you will not be taxed when the benefit is paid to you or your family.

    Income Protection
    The best time to enroll for disability coverage is before you need it.
    If you wait until you experience an illness or non-work injury, proof of good health will be required and there is no guarantee of coverage.
    Optional Short-Term and Long-Term Disability
    Protect your income with Optional Short-Term Disability Insurance and Long-Term Disability. These benefits keep part of your paycheck coming if you become sick or injured and are unable to work for an extended period of time.

    You pay the entire cost of coverage, and since you pay your contributions on an after-tax basis, your disability benefit is not taxable when paid to you.

    Be sure to enroll when you're first eligible. If you enroll later, you'll need to submit a Statement of Health.

    Description
    • Replaces a portion of your salary when you are unable to work as a result of a non-work-related illness or injury
    • If you enroll, your election will remain in effect for two calendar years
    • Replaces a portion of your salary
    • Provides an income to you if you are unable to work because of illness or injury
    • Includes the opportunity to return to work on a trial basis and to participate in voluntary vocational rehabilitation
    Benefit begins
    • On the later of your 8th day of disability or your paid sick time is exhausted
    • The later of you meet the four-month elimination period OR when your salary continuance/sick pay is exhausted
    Benefit pays
    • 50% of your adjusted monthly salary
    • Your adjusted monthly salary is based on your annual base salary or annualized hourly pay – it does not include profit sharing, bonuses, overtime or incentive pay
    • 26 week maximum
    • 50% of your base monthly salary on your last day paid, up to the maximum allowed by federal law
    • The minimum benefit for full-time employees is the greater of 10% of your pre-disability base monthly salary or $100 per month.
    • Severe condition benefit provides a one-time payment of $5,000 if you are diagnosed with a qualifying severe condition.
