Track 1:
Talking with a Health Advocate Coach
  • Call a Health Advocate at 1-888-227-6598 to review your Health Assessment and make your first appointment with a coach who can help you set your goals in managing your back pain and ways to reach your goals.
  • Continue talking to your Health Advocate nurse on a regular basis to help you achieve and maintain your goals.
Track 2:
Using Online Tools
  • Go to MyActiveHealth and choose "Manage My Back Pain" from the Active Online Coaching link (from the left navigation). This will help you set those goals and learn more about ways to reach your goals.
  • Look for your e-mail reminder to check-in with your online coach. Check-in reminders will be sent to you 30, 90, and 180 days after you've started your coaching session. Remember you must complete your check-in within 2 weeks from e-mail reminder at 30, 90, and 180 days.
